Please read the following CAREFULLY before you click “submit”.

You will receive a confirmatory email from TDI Membership within 72 hours with:

Your assigned membership number(s)

Invoice for payment

Attachments for consent and release forms (these are also currently found on the website under “FORMS”)

Attachments for Team Eligibility and Coaching Rosters (these are also currently found on the website under “FORMS”)

Application is NOT considered complete and memberships are not posted until all of the above is received and verified by TDI Membership.

1. All membership mailings and payments should be addressed to:

2. Please help us manage your paperwork efficiently. If at all possible, please send ALL materials in together (consent/release forms, rosters and payments).

3. In order to get proper and prompt credit for payment, ALWAYS put your team name in the memo of your checks or request your school administration to do so, ESPECIALLY if a check is being cut from your district and if there are multiple schools in with teams in your district.

4. Keep copies of all correspondence regarding membership and redeclarations with you at all contests for verification.
